Youth Waiver (Under 18)

Complete this waiver before your appointment so we can keep the in-studio check-in quick and organized. The form saves directly to the studio records once submitted.

What we collect

Participant first and last name, date of birth, and email address
Participant signature and consent acknowledgement
Parent or legal guardian first and last name, and their relationship to the participant
Parent or legal guardian phone number and email address
Parent or legal guardian age certification and signature

I am at least 16 years of age. I am accompanied by a legal guardian for consent purposes. I understand that tattoos permanently change the appearance of my body.

I do not have any medical ailments that will affect the outcome of my tattoo. To my knowledge I do not have any physical, mental, medical impairments or disability which might affect my well-being as a direct or indirect result of my decisions to have any body modification done at this time.

I agree to follow any and all instructions concerning after care of the tattoo or body modification while it is healing. Failure to do so will render any guarantees null and void and as a result any repair work done to the tattoo or modified area will be at my own expense.

I consent to receiving body modification(s) with my agreement on design. I consent to photos of myself and my body modification to be used for promotional purposes and for uses pertaining to Fox and Lamb Tattoo.

I consent to designs made for myself to be used for promotional purposes and for uses pertaining to Fox and Lamb Tattoo (Arielle Racette). I agree to void any monetary gain, fees, intellectual design ownership in regards to body modification or design that I have received with Fox and Lamb Tattoo.

I consent to physical contact in regards to tattoo application or body modification.

Being of sound mind and body, I hereby release Arielle Racette and any person in association with Fox and Lamb Tattoo from all responsibility for any consequences that may arise from my decision to have my body modified.

I hereby release Fox and Lamb Tattoo (Arielle Racette) from all responsibility regarding spelling/grammar mistakes, design anatomy and design accuracy. I understand it is my responsibility to check for any mistakes before the modification process begins and am free to bring up any concerns with the design.

I understand that I have been given the opportunity to dispute or make any changes to the design before beginning the tattoo. I have been given the opportunity to ask any questions about the procedure.

I am aware that some products used during the body modification procedure may be harmful to my health (e.g. allergic reaction).

I agree to not initiate or instigate any type of legal action or defamation against Fox and Lamb Tattoo (Arielle Racette) relating to any and all damages, claims, demands, rights, causes of actions of any kind of nature, based upon injuries, property damage, or death of myself or any other persons arising from my decision to be tattooed or receive any body modification work at this time. I agree for myself, my legal guardian, my spouses, my heirs, my beneficiaries, and legal representatives to hold Fox and Lamb Tattoo (Arielle Racette) blameless of any and all damages, fees, and/or expenses regardless of negligence.

I agree to pay for any and all damages or injuries to any persons in association with Fox and Lamb Tattoo and or property belonging to or used by Fox and Lamb Tattoo caused by or resulting from my actions while I am at Fox and Lamb Tattoo (Arielle Racette).

This release form also pertains to and is designed to protect any and all establishments where Fox and Lamb Tattoo conducts business.

I agree that I have consulted with my physician about any and all of my medical conditions that may affect my body modification prior to having this procedure done. By signing this form I agree that I do not have any diseases or condition, physical or mental, that might affect the healing outcome for my tattoo or body modification. I agree that I do not have any ailments that may be dangerous to any one working in association with Fox and Lamb Tattoo (Arielle Racette).

I agree to inform my artist(s) if any of these conditions apply to me: currently pregnant or breastfeeding, am positive for blood-borne pathogens, epilepsy, known allergies, prone to fainting, consumed alcohol or drugs within the last 24 hours, take anticoagulants, experiencing a bacterial infection or taking antibiotics, skin condition affecting the desired area, any other medical issue pertaining to this procedure.

I understand and have answered honestly to the best of my abilities. Refusal to comply is fraud.

Electronic Signature Consent

By checking here, you are consenting to the use of your electronic signature in lieu of an original signature on paper. You have the right to request that you sign a paper copy instead. By checking here, you are waiving that right. After consent, you may, upon written request to us, obtain a paper copy of an electronic record. No fee will be charged for such copy and no special hardware or software is required to view it. Your agreement to use an electronic signature with us for any documents will continue until such time as you notify us in writing that you no longer wish to use an electronic signature.
